HYBRID reserves the right to change conditions of membership, hours of operation, services offered and all fees and charges, at any time, at its sole discretion with reasonable notice. Any such changes will be notified to you through either the website or mail to your last known email address. 

HYBRID CODE OF CONDUCT: 

By submitting this form you agree to be bound by the HYBRID Code of Conduct. 

MEMBERSHIP FEE DETAILS:

By providing a payment method (credit card) for your membership, you explicitly give HYBRID the authority to charge your membership fee on a weekly basis according to the term of your subscription. Payments are normally processed on a Thursday. If you sign up before Thursday, your first charge will be pro-rated. Total cost of the membership will include a payment processing fee in addition to the above cost. This fee is subject to EzyPay's processing rates and may vary relative to your chosen payment method. All costs include GST. On each occasion that a scheduled payment is declined for any reason, we will continue to attempt to process the payment every two days thereafter. If three consecutive payments fail, the member’s ability to book classes will be restricted. 

COOLING OFF PERIOD: 

There is a seven (7) day cooling off period that begins on the date this form is submitted during which time you may terminate your membership by delivering written notice to HYBRID. The following charges will apply:

(i) All membership fees charged by HYBRID to date. 

(ii) Any payment processing fees paid to date. 

RENEWAL POLICY: 

Flexi Membership: 

Your membership will continue until you cancel your membership with two weeks’ notice. 

6 Monthly Membership:

Your membership will automatically renew month-to-month at the end of the current term at the standard rate unless you cancel at least 14 days before the end of the term. 

CANCELLATION POLICY: 

Flexi Membership: You may cancel your membership at any time with two weeks’ notice after the first month of membership is complete. 

6 Month memberships: 

If you wish to cancel within your minimum term you can do so by paying out your contract – number of months left by half of weekly membership cost. To notify HYBRID of cancellation, send an email to help@hybridtheory.co.nz which details your name, email we have on file for you, cancellation date request and reason for cancelling. 

ACCOUNT SUSPENSIONS: 

6 Monthly Memberships: You may pause your membership for up to a maximum of eight weeks within a 12-month period by providing written notice to HYBRID no less than 5 business days prior. 

Suspensions must start in line with your normal payment collection date (Thursday) but can end on any day.

 Should any payments, fees or other debts remain due but unpaid from any source, HYBRID may suspend your use of the services until all fees and other debts are paid in full.

LIABILITY WAIVER AND INDEMNITY:

HYBRID is in no way responsible for the safekeeping of my personal belongings while I attend class. 

I understand that classes at HYBRID may be physically strenuous and have inherent dangers and risks and I voluntarily participate in them with full knowledge that there is a risk of personal (physical and mental) injury, property loss or death. 

By submitting this form, I agree that I, my heirs, assigns or legal representatives hereby release HYBRID and their owners, volunteers, directors, officers, employees, trainers and independent contractors from any claims of any kind whatsoever for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

I also agree to hold harmless and indemnify HYBRID and their owners, volunteers, directors, officers, employees, trainers and independent contractors from any liability for property damage or personal injury which a third party may suffer as a result of me participating in classes at HYBRID.
